Wegovy vs. Mounjaro: the indication comes before the weight-loss number

Brand names answer a different question from ingredient names. Start with the condition being treated, then match the evidence to the prescription.

The useful finding

Wegovy and Mounjaro have different labeled uses. Tirzepatide beat semaglutide for average weight reduction in SURMOUNT-5, but that does not make Mounjaro the right prescription for every weight-management patient.

Start with the clinical question
GoalWhich condition is being treated?
EvidenceWhich trial matches that question?
OfferWhich product and complete price?

A weight-loss average cannot answer all three questions.

First decide what is being treated

“Which is better?” sounds like one question. For these medicines it contains several: better for weight management, diabetes control, a particular cardiovascular risk, or a routine someone can sustain? A useful comparison identifies the clinical goal before looking at the largest percentage in an advertisement. Otherwise a correct trial result can answer the wrong question.

Wegovy contains semaglutide; Mounjaro contains tirzepatide. In the United States, the current Wegovy label includes weight management and cardiovascular risk reduction in specified populations. The injection also has a defined MASH liver-disease indication under accelerated approval. Mounjaro is labeled for glycemic control in type 2 diabetes, including patients aged 10 and older, and its August 2026 label includes cardiovascular event risk reduction in high-risk adults with type 2 diabetes.[1] [2]

Those qualifications are not small print to discard after reading the brand. For example, “cardiovascular benefit” does not describe one identical eligibility group across both labels. Bring the actual diagnosis and treatment goal to the discussion rather than treating either phrase as a general heart-health claim.

WegovySemaglutideWhich labeled use and which formulation?

Zepbound is the tirzepatide brand with weight-management and specified obstructive sleep apnea uses. Sharing an ingredient with Mounjaro does not erase the product distinction.[3]

Use the direct trial for its actual question

SURMOUNT-5 provides a stronger answer to a weight-loss comparison than putting unrelated trial headlines next to each other. It randomized 751 adults with obesity without diabetes to maximum tolerated weekly tirzepatide or semaglutide regimens. Mean weight reduction at 72 weeks was 20.2% versus 13.7%. The trial was open-label and funded by Lilly.[4]

That finding gives a clinician and patient something substantial to discuss. It does not decide whether a diabetes-labeled product should be prescribed for weight management, whether insurance will pay, or how an individual will tolerate treatment. Nor does it represent every current formulation or regimen bearing one of these ingredient names.

The difference between the reported averages is 6.5 percentage points. That is arithmetic using the rounded published figures, not a forecast of extra weight loss for a particular reader. Someone already receiving semaglutide cannot simply add that gap to their expected result after a switch; the trial did not randomize that scenario.

Our full SURMOUNT-5 analysis explains the study design. If your actual question is about changing an existing prescription, the more useful next read is what switching evidence can and cannot establish.

The direct trial answers a defined efficacy question. Compare care models separately before judging a service offer.

Why older charts still need caution

STEP 1 and SURMOUNT-1 established important placebo-controlled weight-loss results for semaglutide and tirzepatide respectively. They were separate experiments with different treatment regimens and follow-up periods. Neither randomized its participants against people enrolled in the other study.[5] [6]

This distinction survives even when a chart has attractive colors and precise numbers. Subtracting two independent trial averages combines treatment differences with differences between studies. It may be useful as background, but it cannot carry the same interpretation as a direct randomized comparison.

When evaluating a claim, ask for the study name, population and observation period. Check whether the number describes everyone assigned to a treatment or a more restricted analysis. Then ask whether the proposed prescription matches the treatment described. Our cross-trial guide works through this problem without turning it into a leaderboard.

Compare the practical prescription, not just the molecule

The care discussion should produce a specific product and a reason for choosing it. An ingredient name alone leaves too much unresolved: formulation, labeled use, pharmacy route, follow-up and the plan if the prescription changes. Write those items down before comparing prices from different services.

For Wegovy, formulation is especially relevant because the current label covers tablets and injections with different practical routines and some different eligible populations. The oral versus injectable analysis explains why a route preference needs product-specific evidence.[1]

Clinical suitability also involves history and other medicines. Both labels carry thyroid C-cell tumor boxed warnings and contraindications involving personal or family medullary thyroid carcinoma or MEN 2. These are matters for a prescribing clinician, not boxes a comparison website can clear for you.[1] [2]

Ask what the service does when treatment is unsuitable or a change is needed. Does the clinician remain available? Can a shipment be paused? Which charges continue? The answers describe a service relationship; they cannot be inferred from the manufacturer’s efficacy study.

Make the quote match the decision

A complete comparison has two separate conclusions: the clinician’s treatment recommendation and the service’s written financial terms. Keep both visible. An affordable membership may exclude medication, while a medication quote may exclude ongoing care. Comparing only the smallest displayed number can reverse the apparent value of an offer.

Start with our GLP-1 provider shortlist to compare service models. The Form Health review is useful for examining a clinician-led model; it is not evidence that the service is right for every reader or that a particular drug will be prescribed. Use the cost worksheet to record the exact medication, separate care charge and minimum commitment.

Before paying, resolve three questions in writing: what condition the proposed prescription addresses, which exact product the quote covers, and what happens if the clinician chooses a different treatment. If one remains unanswered, the comparison is unfinished. A larger average in a trial cannot fill that gap.

The most useful outcome of reading the research is a more precise conversation. Instead of asking for the medicine with the biggest headline number, ask which evidence applies to your goal and which practical arrangements make the clinician’s plan possible to follow.
